In order to improve the utilization rate of donor organs, Chinese researchers explored the feasibility and surgical methods of transplanting the same donor's heart and lungs to different recipients. The investigators obtained three donors' hearts and two lungs by in situ lavage and one-piece cutting, and were transplanted to three patients with end-stage heart disease and three patients with end-stage lung disease. The donor was mechanically ventilated, then the thoracic artery was opened. The aorta and pulmonary artery were inserted into the perfusion tube respectively. The HTK solution was perfused through the aorta, and the modified raffinose low-potassium dextran solution was perfused through the pulmonary artery to lavage the heart and lungs. Then cut the heart and lungs in one piece. The recipients were separated from the heart and lungs before surgery, and then the retrograde lavage of the heart and lungs was performed again (through the superior and inferior pulmonary vein cannula and the aortic root or retrograde intubation through the vena cava sinus). For unilateral lung transplantation, the left and right donor lungs were separated. Of the 3 lung transplant recipients, 1 underwent left lung transplantation, and 2 patients underwent sequential double lung transplantation without a cardiopulmonary bypass; 3 patients underwent orthotopic heart transplantation with double-venous anastomosis.
The results showed that one patient with left lung transplantation recovered well after operation; one patient with double lung transplantation had a right upper lobe vein embolization, and he was operated again on the 9th day after surgery. The right upper lobe was removed and he was discharged. Another patient with double lung transplantation received postoperative bronchial anastomotic stenosis and pulmonary infection. After symptomatic treatment, he was discharged and discharged from hospital. He died of severe infection 7 months after surgery. One patient with heart transplantation had early renal failure after operation. He was discharged after hemodialysis treatment. Three patients with heart transplantation had good quality of life and heart function recovered to 0-I.
